What happens in Act IV.
Sebastian arrives in Illyria and is immediately mistaken for Cesario. Sir Andrew attacks him, expecting the same timid opponent he faced before, and is shocked when Sebastian fights back. Olivia rushes in, assumes Sebastian is Cesario, and invites him inside. Sebastian is confused but willing. They are quickly married. Meanwhile, Malvolio has been locked in a dark room, declared mad. Feste visits him disguised as a priest, tormenting him further before eventually agreeing to carry a letter to Olivia.

Sebastian is attacked and fights back
Sir Andrew and Sir Toby pick a fight with Sebastian, thinking he is the same Cesario who backed down before. Sebastian defends himself effectively, which shocks everyone and signals that the twin confusion is about to resolve.
Olivia proposes to Sebastian
Olivia, convinced she is with Cesario, asks Sebastian to marry her. Sebastian, bewildered but attracted to her, agrees. They marry almost immediately, which will untangle the love triangle once identities are revealed.
Malvolio in the dark room
Malvolio is imprisoned in darkness and told he is mad. Feste visits him in disguise, switching between his own voice and a fake priestly voice, prolonging the torment. Malvolio manages to pass a letter out, asking Olivia for help.

Sebastian admits privately that he cannot explain what is happening, but since the situation seems pleasant and the woman seems sincere, he chooses to trust it. His acceptance of confusion contrasts with Viola's careful management of hers.

Even from inside a dark room, Malvolio keeps his composure enough to write a coherent letter defending his sanity. This detail matters for Act V, where his grievance will be taken seriously.

The plot resolves itself by accident
Olivia gets the man she wanted, and Sebastian gets a wealthy noblewoman, all because of a mistake. The resolution of the main love plot happens before anyone knows the full truth.
The Malvolio subplot is turning darker
Locking someone in a dark room and telling them they are insane is not harmless fun. By Act IV, the joke has crossed a line, and Malvolio's letter to Olivia is the first sign he will demand accountability.
